2024-2025 OCPS Middle School sports physical packet
Instructions: Create an account on Aktivate.com for your athlete. Fill out the student/parent portion of the packet on the website itself and not pencil paper. Then scan/upload the doctor portion of the 2024-2025 OCPS Sports Physical. Middle School students DO NOT NEED THE ECG as this is for high school athletes only. Once uploaded our athletic director will look over the forms and contact you if anything else is needed.
